For this portion of the database project, students will be comparing two pre-selected healthcare facilities, analyzing deficiencies, and providing recommendations with areas of improvements. Students must create a Provider Deficiency Report (65points) followed by a Recommendation of Opportunities for Improvement (65 points).
Two (2) Word Documents will need to be submitted.

Provider Deficiency Report (65 points)
Analyze the 2 healthcare facilities you selected in Week 4, and Create an Access report analyzing a minimum of 3 deficiencies between the 2 healthcare facilities.
INSTRUCTIONS
1. Create a new query using the Query Design tool.
2. Add Providers, Provider Deficiencies, and Deficiencies tables to the query.
3. Add fields to query:
1. From Providers: Federal Provider Number, Provider Name, Provider State
2. From Deficiencies: Deficiency Prefix, Deficiency Tag Number, Deficiency Description
3. From Provider Deficiencies: Survey Date, Deficiency Corrected, Correction Date
4. Under Federal Provider Number, in Criteria enter the provider numbers of the two hospitals you selected (on separate lines to create an OR condition).
5. Run the query.
6. Save the query as Selected Provider Deficiencies.
7. Capture a screen shot of the query results and paste to a Word doc OR copy and paste to Excel.
8. Create a report based on the Selected Provider Deficiencies query using the basic Report tool.
9. Change the report page layout to Landscape and adjust column widths to fit the report on one page.
10. Capture a screenshot of the report and paste it into a Word Document to include with your submission.
(Your screenshot should show a minimum of 3 deficiencies you selected for each healthcare facility)

Recommendations (65 points)
Recommendations: Create a Word document recommending opportunities for improvement (CQI) for each hospital based on the deficiencies identified.
1. Recommendations should be one page per healthcare facility with top 3 areas of improvement based on legal, privacy and operational areas studied throughout the course.
2. Your recommendations should include the type of trends that would be important to monitor and evaluate.
3. APA format is required including a Title Page
4. Citation page required to justify your analysis. A minimum of 2 sources are required of which one can be your Course E- Book.
