Community Teaching Plan & Evaluation Submission

Create a board or PowerPoint presentation of important learning resources learned throughout the course. Explain approaches to community intervention and evaluation. Implement a teaching and evaluation plan. You will also review and give feedback to the presentation of 2 classmates.

To prepare for the presentation, complete the following steps:

1. Choose one topic from the prevention strategies and intervention recommendations identified in your Community Family Assessment.

· You will be teaching your client from module 4: the family.

· For example – if the family assessment from Module 4 indicated they need to be taught about Diabetes, then that is your topic for this assignment. If they have communication issues, then you teach about effective communication.

2. Develop a teaching plan, including objective, content outline, teaching method, and time in a table format.

3. Develop an evaluation plan for your teaching intervention and create a tool for feedback from your learners.

· For example – Use a questionnaire to confirm they learned the content.

Implement the teaching plan and evaluation during Visit 5 of your clinical experience and then:

· Summarize the outcomes of your teaching plan and evaluation.

· Include your teaching plan, evaluation tool, and summary.

Based on this: My family assessment
Description of the family structure
In my case, I conducted an interview using open-ended questions with an extended family who lives in Miami, Florida. The family comprises four members: the father, the mother, and their two children, a boy and a girl. The father is sixty years old, and a retired truck driver, and the mother is fifty-five years old who happens to be a Registered Nurse. The daughter is twenty-five years old and recently married to G, who lives in Kendall, Florida. All the family members are Caucasian, apart from the daughter’s husband, J, who is African American.
The father is named A, the mother, M the daughter is S and the last born is G. All the family members are Christians apart from A, the father, not a religious member. The mother is the one who had been an active member of the Catholic Church and therefore was the one responsible and accountable for introducing and directing the children to church. Almost more than fifteen years ago, the family conflicted due to A’s irresponsible drinking. The mother works in a nursing home and has a part-time where she provides care services in a private clinic.
A decade ago, the mother was diagnosed with multiple chronic diseases, including depression and diabetes; according to the mother, the depression occurred due to excessive stress from her irresponsible husband at home. Since she was diagnosed with chronic illness, S, the daughters started helping with economical responsibilities in the family. G is currently in college pursuing a nursing degree. Additionally, the responsibility of paying the school fees for children and catering for the family has been neglected by the father and is handled by the mother.
Summary of the overall health behavior
The mother is currently aggressive in ensuring the whole family is living a healthy life. Before the mother was diagnosed with depression and diabetes, the family was mostly taking fast-moving food almost four times per week. Since the mother was diagnosed with chronic illness, she decided to stop taking consuming fast-moving food and decided to provide her family with a balanced diet advised by the doctor. Before her daughter was married, the family had early morning jogging at least three times per week. G, the son, has been advised to take some sports at the college to ensure he is physically fit and can escape chronic illness. S has been physically active and has been participating in the marathon during the local competitions.
However, the father is less concerned about body exercise, and most of the time, he sleeps hungry due to excessive drinking. The family has been sleeping for an average of eight hours since they also have to wake up early for other chores. The mother has to report to her work early, and G has to wake up early to study and attend some home course before traveling to the college for study. Both G and S are very knowledgeable on healthy food when shopping. According to the mother, she wishes that she could have divorced her husband twenty-five years ago when he started irresponsible drinking and a sense of irresponsibility.
Strength and weaknesses of functional health patterns.
The family has excellent strengths in activity health and nutrition patterns (Jiang, Json, Pratte, Beals, Bullock & Manson, 2018). The mother is aggressive in ensuring the family members are taking an active role in the body exercise through jogging in the morning and advising the son to take an active role in sports at the college. Based on the mother, body exercise is a preventive treatment for chronic diseases such as diabetes. Though jogging, the daughter has been able to have an interest in participating in the marathon competition in the community.
The second strength is that the mother has been able to introduce the whole family to a proper diet. The two children can be able to select healthy food items through their mother’s advice. The family also values the importance of balanced food. The main barrier to health is noted with the father, who is not cooperative in exercising his body through jogging or walking. The father is vulnerable to contracting chronic diseases such as diabetes. The father is also susceptible to liver cirrhosis as a result of excessive alcoholism. The aspect that the mother is suffering from chronic diseases is a barrier to health. Most of the resources will be diverted in the health provision instead of providing the family with proper dietary.
The family genogram
A family genogram is a graphical representation of the family, which can also be in a tree diagram (Sitnik-Warchulska & Izydorczy, 2018).
